Many users access the Portal in their capacity as a workforce member of a Business Associate (such as a law firm, billing partner, or other authorized organization) under HIPAA. By accessing the Portal you confirm, on behalf of your organization, that:
- Your organization has executed a Business Associate Agreement (BAA) with Regenerative Pain & Spine, or is a workforce member of an entity that has done so, OR your organization is a covered entity treating the same patient under HIPAA Treatment, Payment, and Healthcare Operations (TPO).

Every authentication event and every access of Protected Health Information through the Portal is logged in an append-only audit record that retains, at minimum: timestamp, user identifier, user role, source IP, action taken, resource accessed, and outcome. Audit records are retained indefinitely.
Regenerative Pain & Spine reviews audit records periodically and on demand. Anomalous access patterns may trigger automated alerts, account suspension, and a written explanation request to the user and their organization. Filing a false explanation, or tampering with audit records, is grounds for immediate termination of access and may be reported to regulatory authorities and to the user's organization.
